STEP 1 - Documentation collection

CLINICAL DOCUMENTATION

Prepare all the clinical documentation, officially translated into English or Italian, describing the history of treatments performed from the onset of the disease to the current state of health.  The Second Opinion Service is based on the information sent, so it is essential that the documents are accurate and updated as much as possible.

The following documents are required for a proper assessment:

  • A recent report from the referring oncologist (dated no more than 30 days prior to the date the documentation is submitted).
    Important: The referring doctor’s report must contain all the clinical information necessary to provide an appropriate opinion: diagnosis and stage of the disease, non-oncological medical history, detailed oncological medical history, chronology of treatments carried out (including dosages of oncological therapies administered), current treatments, home medication, active problems and the clinical query for the IEO Second Opinion Service.
  • Available histopathological reports (molecular biology reports if available);
  • Radiological reports;

Please note: it is not necessary to send copies of blood test results (unless explicitly requested by the IEO doctor).

Should further documents be required, the IEO Second Opinion Office will request that they be sent.

ADMINISTRATIVE DOCUMENTATION

In addition to the clinical documentation listed in the previous paragraph, to request the IEO Second Opinion Service, you must download and complete the following forms:

  • General Information;
  • Service Request Form
    • It is necessary  filling in the "Main Contact" field only if the service applicant is a different person from the patient. In this case the following documents are required:
  • An identity document (ID or Passport) of the person requesting the Service on the patient's behalf
  • A document proving the relationship with the patient if the applicant is:
    • Holder of the documentation, adult or minor;
    • Parent of child with parental authority (family);
    • Entity/individual acting as ward (providing proof)

STEP 2 - Request submitting

Once all the clinical and administrative documentation referred to in the preceding paragraphs has been prepared, all documents must be sent to the following email address: second.opinion@ieo.it

We kindly ask you to send a single email with the following details:

  • Subject line: Patient’s first name, surname and date of birth;
  •  Attachments:
    • Clinical documentation

To ensure the documents you send are correctly identified, please send the attachments in PDF format and name them appropriately, using keywords that describe their content.
Here are some examples:

  • Histological examination: "Histological examination"
  • CT Scan Report: "CT Scan Report"

In the case of reports from your own treating oncologist, we also recommend sending the document (in PDF format) typed on a computer so that it can be read immediately by the IEO doctors who will be responsible for the patient’s second opinion.

STEP 3 - SERVICE PAYMENT

Once the clinical documentation has been reviewed by the IEO triage doctors, the patient or the person requesting the service will receive confirmation within two working days from the email address second.opinion@ieo.it that the documents submitted are complete and correct.
